A progress report is a clinical documentation record used to track a patient’s progress throughout an ongoing therapy case (episode). It captures objective improvements, therapeutic interventions, and clinical reasoning between evaluation and discharge events. Progress reports help demonstrate medical necessity, justify continued care, and guide updates to the treatment plan.
