Evaluation

An evaluation is the first certification within a therapy case. It establishes the clinical foundation for treatment by documenting the patient’s condition, medical history, therapy goals, and plan of care.

Evaluations serve as both the initial assessment and the first certification period, capturing baseline information that supports the medical necessity of ongoing therapy. Subsequent recertifications build on this information as treatment progresses.


Data model

Every therapy case has one or more certifications. The first certification is designated as the evaluation while the subsequent certifications are designated as recertifications.

Each certification maps to 0 or more treatments, a therapist, and an admission.

Discussion

In RehabAlpha, the evaluation is created alongside a therapy case. An eval cannot exist without a case and vice-versa.

The evaluation collects clinical information about a patient, including

  • medical and treatment diagnoses
  • past medical history
  • medications
  • measures
  • therapy necessity
  • CPT codes
  • the evaluating therapist

Futhermore, every evaluation requires a start and end date that defines the window of time for which treatment will be provided.

When you set up a new case, you can exclude a treatment plan. In such cases, the evaluation's end date will match its start date.

The evaluation remains editable until it receives a signature or signature request.
